During your initial visit to our clinic, we ask that you please bring your insurance information with you. If you have health insurance that covers all or part of the cost of your vision correction treatment, we will file a claim directly to your insurance company. We accept assignment from Medicare and BCBSND. Patients are responsible for the deductible, co-payment, and service fees that are not covered by insurance. Patients who are members of a managed care health plan are responsible for bringing all the necessary referral forms during their scheduled appointment. Co-payments are collected upon check-in.
For all surgical claims, our office will file the claim with your insurance carrier. We must receive the proper mailing address and policy information from patients in order to submit these claims correctly.
Short term financing options are available through Care Credit. Qualified applicants (qualification determined by Care Credit) become eligible for financing options by completing the Care Credit application. Follow the link below or call the office at 701-255-7775 to inquire about options.
