APPOINTMENT SCHEDULING
All
appointments are scheduled through our main clinic office
at 701-255-4673, or toll-free 800-472-2620. Our scheduling
staff will need to know the nature of your medical eye problem.
Information such as your telephone numbers, addresses, social
security number, insurance information, and if you have been
referred to us by another doctor, etc., will be asked at the
time you schedule your appointment.
It
is our sincere intention to make sure that you are seen at
the time designated, and we will always try to keep any waiting
to a minimum. We know your time is important.

BILLING/INSURANCE/FEES
Our primary concern is for the health of your eyes. Our fees
for services are billed on a monthly basis through our billing
office. For any billing or insurance questions, please call
701-255-7775 or toll-free 800-472-2620.
On your initial visit to our clinic, please bring your insurance
information with you. If you have health insurance that covers
all or part of the cost of your treatment, we will file a
claim with your insurance company. We accept assignment from
Medicare and Blue Cross Blue Shield of ND. Patients are responsible
for the deductible, co-payment, and services that are not
covered by insurance. If you are a member of a managed care
health plan, it is your responsibility to bring the necessary
referral form. Co-payments will be collected upon check-in.
On all surgical claims, our office will file your claim with
your insurance carrier. We must receive the proper mailing
address and policy information from you in order to submit
these claims correctly.
For
your convenience, we accept Visa, Mastercard, and Discover.
